Clinical Manifestation

1. Episcleritis

The clinical features of episcleritis include rapid onset, with a disease course lasting about 10 to 20 days. During the period of the initial onset, there are symptoms of discomfort in the affected eye including photophobia, tearing and pain. The affected region shows redness and localized edema in the superficial layer of sclera. There can also be localized nodular bulging which presents itself as dark red, slightly movable, and appears in multiple quadrants, due to the recurrent attacks in different parts of the sclera. The disease has a partial self-limiting tendency, but recurrent attacks can last over a period of many years.

2. Scleritis

The clinical features of scleritis include rapid onset or gradual progression, severe pain in the affected eye, radiating pain to the affected side of the head, and edema in the conjunctiva. Hyperemia shows dark red or prunosus in the superficial as well as the deep layer. Single or multiple nodules may present in the sclera that are hard and immobile with tenderness. Some severe conditions may develop rapidly, or become complicated forms of uveitis. If vasculitis occurs in the outer vesicles of affected sclera area, some features may appear in the lesion or the surrounding area including non-perfusion capillary, infarction and necrosis, thinning and light blue coloring of the involved sclera. Severe scleritis may bring about staphyloma or perforations of the sclera.
